Case Manager - Full Time

PRONIA is seeking a suitable applicant for the Case Manager position on a Full-time basis. Reporting to the HCP Coordinator, you will be responsible for providing support to Home Care Package clients, monitor care recipient needs, seek opportunities for growth in the Home Care Packages program and undertake additional tasks in line with the overall position relating to PRONIA.

The successful applicant will have, amongst other attributes, appropriate tertiary qualifications preferably in Social Work, nursing or allied health fields, minimum 1 year experience in community aged care or case management, awareness of issues for Greek speaking carers and ability to work cooperatively and autonomously within a team environment.

Proficiency in speaking the Greek and English language, with excellent written and oral skills and excellent time management are essential. Proficient use of Microsoft suite and packages such as Word, Excel, Access, Internet and E-mail are also required. Also, you will need to have the capacity to work from home if another lockdown will enforced, owning your own computer and having internet access at home.

Community Engagement and Support Practitioner(COVID-19)- Part Time (Fixed Term)

PRONIA is seeking a suitable applicant for the Community Engagement and Support Practitioner (COVID-19) position on a part time – 6 month fixed term basis. Reporting to the Client and Community Services Coordinator, the role aims to enhance wellbeing and support to youth and women impacted by COVID 19 via the provision of direct casework (including outreach), emergency relief, and access to a range of services and to enable employment and art workshops.

The successful applicant will need to have tertiary qualifications preferably in Health, Welfare or Human Services field with a minimum 2 years’ experience in Social Welfare field, awareness of the needs of Australian –Greek community, proficiency Greek and English language, with excellent report writing skills and excellent time management.

HCP Team Leader - Full Time

PRONIA is seeking a suitable applicant for the HCP Team Leader position on a Full-time basis. Reporting to the HCP Coordinator, you will primarily work with internal and external stakeholders as well as external contacts such as state and commonwealth government departments. Including Aged Care Assessment Teams, Services Australia, My Aged Care and the Aged Care Quality & Safety Commission. Also, the HCP Team Leader will be responsible in supervising the Case Managers in the HCP Program.

The successful applicant will have, amongst other attributes, appropriate Diploma or higher qualifications in Social Work, or related human services field, minimum 3 years of experience in staff supervision in the aged care or disability sector, awareness of issues for Greek speaking carers and ability to work cooperatively and autonomously within a team environment.

Community Care Worker - Casual & Permanent Part Time

PRONIA is seeking to recruit people to join their vibrant and dynamic Aged & Co-ordinated Care Services Team.

We are looking for people who are interested in providing support to elderly and people with disabilities in the community to work in our Home Care Packages:

  • Home Care: House cleaning (e.g. dusting, wiping, mopping etc.) and other tasks such as washing, ironing, cooking and shopping.
     
  • Personal Care: Provide assistance for their personal care (e.g. helping with food, assistance in dressing, showering, toilet etc.), as well as accompanying them to appointments or leisure activities.

We are looking for people who currently have a CERT III in Home and Community Care or equivalent or willingness to do the course whilst in paid employment as a Community Care Worker.

Reporting to the Manager, you will work in accordance with Children’s Centre policies and procedures; ensure a safe environment is maintained for both staff and children. The successful applicant will have, relevant tertiary qualifications and demonstrated experience in Early Childhood development and care. Understanding and appreciation of cultural diversity and issues affecting the CALD community.
