What you will be doing
In this role, you’ll work closely with older adults to coordinate personalised care plans that support their ability to live safely and independently at home. You’ll conduct assessments, link clients with appropriate services, and regularly review their needs to ensure quality care. As a key advocate for your clients, you’ll collaborate with families, service providers, and other professionals to promote wellbeing and dignity. You’ll also maintain accurate documentation and ensure compliance with aged care funding and reporting requirements, all while supporting clients from diverse cultural backgrounds with empathy and respect.
The role is crucial to the growth and the implementation on strategic growth.
Key Responsibilities
- Respond to calls appropriately within scope of role and business rules and escalate accordingly
- Ensure accurate documentation within the Client Management System (Visual Care) to reflect any changes
- Provides basic coordination support to care recipients on Home Care Packages and refers to case managers for case management needs
- Develop and maintain relationships with service providers to ensure accurate and timely service coordination
- Liaise with multiple service providers to ensure care recipient needs are met in line with objectives and timelines
- Demonstrates an understanding of the target population and their needs from a service coordination perspective
- Monitor and accept client intake referral requests and support the Intake and Interim Case Manager positions
- Provide relevant and timely feedback to Case Managers or Care Partners relating to client or provider issues
- Respond to PRONIA client queries and make changes to client service delivery within scope
- Ensure all information is accurate and maintained relating to client service delivery and enquiries
- Undertake service coordination as defined by the role
- Meet regularly with the Service Coordinator and Intake Team Leader to provide reports as required
Role Requirements
- Qualifications in relevant customer service related discipline (welfare, administration, CERT IV, etc.).
- Experience in administration duties no less than 2
- Fluency in Greek and English
- Proficiency in the use of Microsoft Office Suite and packages such as Word, Excel (Intermediate Level Mandatory and Advanced preferred) Access, Internet and E-
- Knowledge of client specific software applications including but not limited to Visual C highly
- Understanding and or willingness to train in use of My Aged Care (MAC) Provider Portal and Medicare Online Claiming Portal.
- Demonstrated commitment and experience in providing high quality customer service.
Mandatory Requirements
- Driving License
- Access to own vehicle
- Undergo Criminal History Record Check at the required intervals
- Not subject to any Employee Screening or Banning Orders under relevant state or federal legislation (e.g., Worker Screening Act 2020 (Vic)).
